
At Florida Functional Neurology Group (FFNG), we don't just hand you a clipboard and call it an intake. For us, this process is a crucial diagnostic tool. We specialize in complex neurological cases like traumatic brain injury (TBI), concussions, autoimmunity, and dizziness. We know how frustrating it can be when you feel terrible, but your standard imaging keeps coming back as "normal."
That’s why we dive deep into your clinical history before you even walk through our doors. This prep work ensures that your actual exam is focused entirely on finding the exact neurological and metabolic network failures that are driving your symptoms.
We want to make the most of every minute you spend face-to-face with our providers. To do that, we use what we call a "Files First" protocol. Basically, we ask that you complete and return all your intake documents BEFORE we schedule your initial evaluation.
Why?
Because reviewing your history ahead of time gives our clinical team the chance to make sure you are a great candidate for our advanced, drug-free neurological care. Please gather and prepare the following items for your submission:
Intake Forms: Completed digital forms (links below).
Medical Records: Significant past reports and physician diagnoses.
Imaging: X-rays, MRIs, and CT scans with their written reports.
Testing: Recent NCV/EMG results and blood panels.
What to wear: Throw on some gym shorts and a t-shirt. We'll need to measure your uninhibited movement and stability, so you'll want to be comfortable.
Time: We don't rush this. It’s a thorough evaluation that takes time, so please bring a light snack!
